Patient-Generated Health Data

This measure reports the percentage of patients that meet both of the following:

  • They sent a secure message with an attachment to their provider during the measurement period.
  • The provider saved the message attachment to the patient’s chart and indicated that it is Patient Generated Health Data (PGHD).
Measure Overview
Denominator The number of unique patients who had at least one non-voided face-to-face visit with an eligible provider or clinician during the measurement period set in Prime Suite Reporting 2019. This visit must have been checked in during the measurement period. Additionally, the best practice is to also check-out the visit during the measurement period.
Numerator The number of patients who supplied Patient Generated Health Data via the patient portal which was then added to the patient’s chart in Prime Suite. These actions must occur during the measurement period.

Denominator Criteria

Patients are counted in the denominator if they had at least one non-voided face-to-face visit during the measurement period set in Prime Suite Reporting 2019. This visit must be with an eligible provider and checked-in during the measurement period.

Numerator Criteria

Patients who qualified for the denominator count in the numerator if both of the following occurred during the measurement period set in Prime Suite Reporting 2019:

  • The patient sent a message with an attachment via the patient portal to the provider.
  • The message attachment was saved to the patient’s chart and marked as Patient Generated Health Data.

When importing a document from a patient message, you must select the Chart radio button to import to the patient’s chart. Using the Administration option will not qualify the patient for the numerator.

To meet this measure, you must have purchased and implemented the Greenway Patient Portal. If you do not currently have patient portal functionality, please contact your sales representative.

Patient-Generated Health Data Workflows

Patient portal messages go into Prime Suite as tasks. These tasks are typically assigned a task type that begins with Patient Portal. Examples of these task types include Patient Portal General Question, Patient Portal Question for Doctor, and Patient Portal Prescription. The sections below provide more information on:

Capturing PGHD from a Secure Message from a Patient

1. Select Open Task List from the Actions menu to open the My Tasks window.

2. Select and open the appropriate task.

The presence of the Associated Documents link at the top of the message indicates that the message contains attachments.

3. Select the Associated Documents link to open the View Associated Documents window. Attachments are found under View Associate Reference Documents at the bottom of the window.

4. Select View next to the attachment to open the Reference Document Viewer window. The options at the bottom of the window allow you to set parameters for the import.

5. Select the Chart option to import to the patient’s chart.

Using the Administration option will not qualify the patient for the numerator.

6. Select the document type in the Type list.
7. Adjust the Date field, if needed. The date automatically sets to the date the document was received.

Regardless of the date set in this field, the date the document is added to the chart is used to determine whether the item qualifies for the measure.

8. Update the name of the document, if needed.
9. Select the Contains patient-generated health data check box. This indicates that the attachment contains clinical data that was generated by the patient and is required to qualify for the numerator.
10. Determine whether the document has been reviewed by a provider.

Been reviewed
1. Select the Signed/Authenticated check box to indicate that the document was reviewed by a provider.
2. Complete one of the following to record who signed the document:
  • If the document was reviewed by a provider in your practice, select the By option and then select the provider's name in the By list. This provider will be credited with the qualification.
  • If the document was reviewed by someone outside of your practice, select the Other option and then enter the name of the person who reviewed the document in the Other field. This option will not qualify the patient for the numerator.

Select the Assign To option and then select the provider who should review it in the Assign To list.

The document will be placed on the provider's Unsigned Documents list. The provider that the document is assigned to will be credited with the patient’s qualification.

Select the High priority check box to mark the item as a high priority item.

3. Select Import to import the document.

After importing the document, you can view the document in two places. You can either select View Patient’s Document in the Patient Information Bar in Prime Suite or the Document link in Patient Chart. Additionally, you can change whether a document is indicated as containing Patient Generated Health Data (PGHD). See Modifying the PGHD Label on a Document After Import below for more information.

Modifying the PGHD Label on a Document After Import

1. Select the View Patient’s Document link in Patient Information Bar to open the document.
2. Scroll to the footer of the document to view the options.

This footer only displays when you open the document from the View Patient's Document link in the Patient Information Bar.

3. Select or clear the Contains Patient-Generated Health Data check box to indicate whether or not the document contains PGHD. Changing this setting changes the Close Note button to a Save button.
4. Select Save to save the changes and close the window.

If you clear the Contains Patient-Generated Health Data check box and save the document, the patient will no longer qualify for the numerator.
